PHMC requires that all employees are fully vaccinated with the Covid-19 vaccine before the first day of employment.Interim House, a comprehensive substance abuse program for women located in West Mt. Airy, Philadelphia, PA, is seeking an individual to serve as our Caseworker. The Caseworker is responsible for carrying a caseload of participants in the residential/halfway house and Outpatient treatment program. Responsibilities include benefit activation, monitoring outside services, implementing aftercare referrals, accompanying women to community appointments as assigned, acquiring identification, supervising visitation, ensuring aftercare plans are completed, and serving as a liaison to the criminal justice system. The position also includes clerical and administrative functions in IHI’s residential, halfway house, and outpatient programs as needed, including support for client authorizations for admissions and related tasks.This individual will work closely with the Clinical Director, residential and outpatient staff, as well as the program director.QUALIFICATIONS

MINIMUM:Associates Degree in related field and one year experience OR High School Diploma and five years’ experience in a substance abuse setting.Excellent communication and organizational skills.Knowledge and understanding of co-occurring disorders, including trauma/abuse and substance use/abuse.Capacity for relating to women in an open and supportive way.Awareness of the special needs of women in recovery.DUTIES:Initiate steps to acquire client identification, including social security card, picture ID, and birth certificates for each program participant.Submit supplemental paperwork to DPW to ensure all client benefits are activated in a timely manner and obtain SAR number to ensure benefits are maintained and client has no disruption in funding.Collect client food stamps via the EBT machine and provide weekly tracking report to supervisor for case review discussion.Utilize motivational interviewing techniques and solution-focused problem-solving skills to assist participants with resource needs.Accompany clients to check cashing places to have checks cashed; instruct on how to complete money orders when necessary or personal check writing.Maintain an individual file on each client that contains up-to-date financial information.Check EVS daily for RES, HWH, and OP and provide a copy to Admissions Coordinator and Clinical Director for follow-up.Program Coordination:Assist Admissions Coordinator in intake process and/or initial screening calls, when needed.Perform new client clothing inventory when needed and utilize the donation inventory.Ensure all consents and releases are signed and witnessed following documentation protocol.Accompany clients to court if needed and provide court personnel with progress documentation.Pick up clients from county jails and/or other treatment providers.Transport clients to other treatment providers upon discharge.Transport clients on caseload to needed appointments if assigned and cover for other staff when transportation is needed.Accompany clientele to weekly off-site 12-Step meeting, as needed.Liaison to legal entities (PO’s, Bail Agencies, Project Dawn Court, Recovery Court, etc.) and IHI therapists; obtain weekly Progress Report from therapists at Case Review. Complete criminal justice reports and submit to legal entities.Ensure clients maintain a list of pertinent contact information/phone numbers such as DPW worker, probation officer, FIR case manager, physicians, etc.Complete Residential Preliminary Aftercare Plan within one week of admission.Complete Continuing Care Plans for all residents two weeks prior to discharge. Ensure MAT follow-up appointments are scheduled to ensure continuity of medication.Assist Intensive Outpatient Program clients with housing aftercare planning when needed.Attend weekly community meeting with all staff members and the community of clients; address relevant case management concerns which arise during the meeting.Complete One Week Follow Up Form for all discharged clients to confirm initial aftercare appointment was attended by participant.Assign locker to each resident. Verify contents of locker with resident via locker inventory sheet. Ensure client is given contents at discharge and document they were provided.Create weekly Case Review Agenda.Support resource community outreach events and obtain community resources relevant to the participants' aftercare planning.General, Administrative & Other Duties:Adhere to yearly training plan.Print out monthly record of service for IOP/OP clients.Create weekly case review schedule.Support coverage if needed at front desk.In absence of admissions coordinator, provide coverage if assigned.Alternate as needed with RES/IOP/OP filing and opening and closing RES/IOP/OP charts.Other duties as assigned by supervisor or program director.Assist with community outreach events.SALARY:
