Nurse Practitioner Psychiatric Mental Health

Seeking a full-time nurse practitioner to join Providence ElderPlace's Program of All-Inclusive Care for the Elderly (PACE) in Portland, Oregon. This program provides comprehensive care, social services, psychiatric care and acute care to geriatric patients across the Portland Metro area. As a member of this team, you will be providing these services in patients homes as well as in medical clinics. You will also work closely with LCSW Mental Health Case Managers while helping to integrate behavioral health services into the program as well.
