Providence ElderPlace Oregon is seeking a Geriatric Physician for an On-Call/Per Diem position at Providence ElderPlace. You'll join an established interdisciplinary team providing older adults with a single, coordinated program of comprehensive care and social services. Be a part of an exceptional team of caregivers passionate about caring for older adults.Key Responsibilities
On-Call/Per Diem (no guarantee of hours)Collaborate with nurse care managers, social workers, pharmacists, mental health case managers, and other team members.Interest in obtaining MAT Waiver for prescribing Suboxone.Join a team dedicated to Diversity, Equity, and Inclusion.Qualifications
Board-certified/board-eligible Family Medicine or Internal Medicine trained physician, or completion of Geriatric Medicine fellowship.Where You'll Work
Providence ElderPlace is a Program of All-inclusive Care for the Elderly (PACE), and is the first program of its kind in Oregon. With eight conveniently located centers in the Portland area and on the Oregon Coast, Providence ElderPlace provides and coordinates all necessary care services for seniors to remain active within their communities.
