What You Will Do to Change LivesCoordinates service needs and collaborates with other services and agencies. Completes assessments and determines the appropriate level of services for new members served.Shifts Available:Full Time; AM 8:00 am - 4:30 pm varies as needed; Monday - FridayExpected starting wage range is $32.24 - $39.83. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed.
