Patients may range in age from birth to over 100 years. Collaborate with patient, family, physician and other allied health professionals to identify patient needs and coordinate services to meet those needs.Key Activities
Develop a plan of care that is patient and family focused and institutes standardized interventions for goal oriented outcomesDaily documentation is accurate and timelyApplication of standard treatment proceduresTeaching is provided to patients and caregiversQualifications and Experience
One year experience in acute care, rehab or homecare as a Physical TherapistRegistered with the Board of Registration in Massachusetts.Strong customer service skills and ability to work with diverse populationsExcellent verbal, written and electronic communications skillsComputer proficiency and ability to learn new softwareAbility to prioritize work and adapt to changing situationsTravel required within Mass General Brigham Healthcare at Home's geographic area
