You can do all this and more at UCLA Health.You will provide a wide range of services as part of a multidisciplinary team, with an emphasis on outpatient practice within the UCLA Primary Care Network. This will involve serving diverse child, adolescent, adult, and geriatric populations. You will be responsible for psychosocial assessments; care coordination; team consultation; individual, family, couples, or group psychotherapeutic treatment; serving as a liaison with community agencies; treatment plan documentation; and disposition planning and advocacy.UCLA Health is a world-renowned health system with four award-winning hospitals and dozens of primary care practices, specialty practices, urgent care centers and other ancillary locations throughout metro Los Angeles. Qualifications

We're seeking an experienced CA Licensed Clinical Social Worker with:Master's degreeExperience collaborating with healthcare providers in a multidisciplinary settingWorking knowledge of general theoretical and clinical modelsAbility to develop rapport with patientsExcellent interpersonal and communication skillsAbility to document/chart to ensure current and accurate medical recordsKnowledge of social work ethics and scope of practiceAbility to perform a complete mental exam and recognize changes in mental statusKnowledge of relevant community resourcesProficiency in coordinating care with providers and community agenciesAbility to serve as a patient advocate with assertiveness, persistence, and diplomacy
