UCan do all this and more at UCLA Health.In this key role, you will provide a wide range of services in support of multidisciplinary teams with an emphasis on outpatient practices of the UCLA-Community Physicians Network (UCLA-CPN). The Network currently serves adult and geriatric populations and may also serve child and adolescent populations in the future. You will be responsible for:Psychosocial assessmentCare coordinationConsultation to teamIndividual, family, or group psychotherapeutic treatmentLiaison with community agenciesTreatment plan documentationDisposition planning and advocacyMultiple locations available, including Santa Monica, West LA, Marina Del Rey, Brentwood, and Calabasas, as well as flexibility in schedule.Position is flexible hybrid with up to 40% remoteSalary range: $64,800.00-$137,600.00 AnnualQualifications

We're seeking a self-motivated, compassionate individual with:Appropriate California License and Degree (LMFT, Master's in Marriage & Family Therapy, Clinical or Counseling Psychology)Experience interacting with health care providers in a multidisciplinary settingWorking knowledge of general theoretical and clinical modelsAbility to develop effective rapport and working therapeutic alliances with patientsSkill writing clear and complete clinical notes and summaries of clinical evaluationsSkills and experience documenting/charting to ensure current and accurate medical records are entered and documented in patient files in accordance with established Medical Records policies and proceduresKnowledge of social work ethics and scope of practiceAbility to perform a complete mental status exam and recognize changes in mental statusAbility to establish and maintain effective and mutually respectful relationship with professionals and support staffKnowledge of pertinent current community resources, including income maintenance, mental health treatment, special education, residential and vocational supportsAbility to provide and maintain appropriate coordination of care with existing providers and/or a variety of community agencies and demonstrate assertiveness, persistence, and diplomacy as an advocate for the patient
