As an RN Home Health, you will play a crucial role in providing comprehensive home care services to patients and their families. You will have the opportunity to make a real difference in the lives of our patients by helping them achieve their goals for staying safely at home.Overview:The home health registered nurse plans, organizes and directs/coordinates home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.Duties & Responsibilities:Develop and initiate the plan of care in partnership with the patient, representative, and caregiver(s)Provide preventative and rehabilitative nursing procedures as ordered by the physicianOffer patient, caregiver, and family counselingDeliver patient and caregiver educationPrepare clinical notes and communicate with the physician and other healthcare practitionersIdentify discharge planning needs and implement prior to patient dischargeCommunication:Prepare clinical notes and update the primary physician as necessaryCommunicate with the physician regarding the patient's needs and report any changes in the patient's conditionCoordinate care plan with community health-related personsAdditional Duties:Participate in the HHA's quality assessment and performance improvement programParticipate in on-call duties as defined by the on-call policyEnsure availability of necessary equipment and servicesInstruct, supervise, and evaluate home health aide care provided every two weeksSupervise Licensed Practical Nurses (LPNs)/ Licensed Vocational Nurses (LVNs) and/or Home Health Aids (HHAs)/Certified Nursing Assistants (CNAs), as directedJob Requirements:Graduated from an approved school of nursing and licensed in the state of practiceMinimum of two years of experience, with at least one year in public health or home care nursing preferredCurrent CPR certificationLicensed driver with an insured automobile in good working orderSelf-directed and able to work with minimal supervisionExcellent observation, problem-solving, verbal and written communication skillsAbility to organize and prioritize workload independentlyNo management experience required, responsible for supervising home health aidesAble to perform required nursing skills, including lifting, positioning, and transferring patientsBenefits:Flexible schedulePaid time offHealth insuranceDental insuranceVision insuranceLife insurance401k matchingEmployee discountMileage reimbursementLocation: San Antonio AreaIf you are a talented RN with a passion for providing exceptional home health care, we encourage you to apply now!
