Join our team and be part of a company that puts its employees first.Job Summary:The registered nurse plans, organizes, and directs hospice care, with an emphasis on community health education and experience. As an RN Hospice, you will utilize your nursing skills to provide professional care to patients and their families within their homes and communities. Your responsibilities will include completing comprehensive assessments, developing care plans, administering medications and treatments, coordinating patient care, and providing emotional support to patients and their families.Duties & Responsibilities:Patient Care:Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needsProvide professional nursing care utilizing the nursing processDevelop care plans based on nursing diagnosis and incorporate palliative nursing actionsAdminister medications and treatments as prescribed by the physicianAssist patients with activities of daily living and facilitate self-sufficiency and comfort careAct as a case manager and coordinate patient careCommunication:Maintain accurate and relevant clinical notes regarding patient's condition and careCommunicate with physicians and community health-related persons to coordinate the care planTeach patients and families self-care techniques and provide health counselingProvide a safe environment for patientsWork in cooperation with the interdisciplinary group to meet emotional needs of patients and familiesAdditional Duties:Participate in on-call duties as defined by the on-call policySupervise ancillary personnel and delegate responsibilities when requiredMaintain and upgrade professional knowledge and practice skills through continuing educationActively participate in quality assessment performance improvement teams and activitiesJob Requirements:Graduate of National League for Nursing accredited school of nursingMinimum of two years of experience, with at least one year in public health, home care, or hospice nursing preferredRegistered nurse with current licensure to practice professional nursing in the stateDemonstrates excellent observation, problem-solving, and communication skillsAbility to organize and prioritize workload independentlyMaintains a current CPR certificationMust be a licensed driver with an insured automobile in good working orderSelf-directed and able to work with minimal supervisionPerks and Benefits:Flexible schedulePaid time offHealth insuranceDental insuranceVision insurance401k matchingEmployee discountMileage reimbursementLocation: Kingman, AZIf you are a highly skilled and motivated registered nurse with a passion for providing exceptional care, apply now and join our dynamic team at River Valley Home Health and Hospice.
