Job Description

Atlantic Visiting Nurse, part of Atlantic Health System, is seeking a per diem Registered Nurse to work Monday - Friday from 8 AM to 4 PM in the Sussex / Warren County area, including weekends and on-call as required by department. In this position you will work with adult and geriatric patients of all backgrounds who require intermittent skilled short-term care in their homes. You are compassionate, thoughtful, and comfortable working with elderly and terminally ill patients. You will be responsible for managing a heavy caseload; time management skills are essential. However, it is important to provide each patient with the time and attention necessary to meet his or her needs.

Entry level offers start at $43.00 / hour and increase based upon experience. Mileage reimbursement ($.67) included.

Essential Job Functions:Physical assessment of patient providing MD ordered treatments, extensive patient education, frequent communication with providers.Candidate will have the ability to utilize the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served. Through the model of Shared Governance, collaborate with all members of the health care team with a focus on promoting evidenced based care within a healing culture for the patient population served and their families.Candidate will demonstrate and embrace the PRIDE core values (Professionalism, Respect, Involvement, Dignity and Excellence) in every aspect of their job.Perform evaluation tasks, including vital signs and medication review.Administer medication as prescribed by the patient's physicianDress or undress open wounds and assess progress of healingEducate patients and families on proper home health care strategies and proceduresCoordinate with Occupational Therapists, Physical Therapists, Physicians and anyone else involved in the patient's care planProvide palliative care as needed to keep patients comfortable.Make recommendations for devices or tools that might improve the patient's quality of life.Listen to the patient and respond to concerns or requests.Education:

Graduate from an accredited RN program, BSN preferred

Valid New Jersey RN License

If you and your physician feel that you would benefit from home care - whether to prevent or postpone a hospitalization, or to help with recovery after a hospital stay - our team of nurses and rehabilitation experts can develop an individualized plan of care to meet your needs. We're accredited by the Joint Commission and licensed by the New Jersey Department of Health. Atlantic Visiting Nurse - Home Health services are available to residents of Morris, Union, Essex, Sussex, Somerset and Warren counties in New Jersey.

Hospice

At Atlantic Visiting Nurse, we believe that those living with advanced illnesses should spend their final days in comfort and peace. Our compassionate, specialized care is designed to minimize patient suffering and to support families and loved ones in their roles as caregivers. As one of the first hospices in New Jersey and the United States, we have more than 40 years' experience serving private homes, nursing facilities and assisted living facilities in Essex, Morris, Somerset, and Union counties. When patients and their primary care physicians agree that curative treatment is no longer appropriate or desired, a referral to Atlantic Hospice may be appropriate. Our hospice team works with referring doctors to develop individualized care plans that manage pain, control symptoms, and ensure dignity and respect in the final days of life.
