Job Description
Atlantic Visiting Nurse, part of Atlantic Health System, is seeking a full-time hospice Registered Nurse to work Monday - Friday from 8 AM to 4 PM in the Morris, Essex, Union, and Somerset County area, including weekends and on-call as required by department. In this position you will work with adult and geriatric patients of all backgrounds who require care in their homes. You are compassionate, thoughtful, and comfortable working with elderly and terminally ill patients. You will be responsible for managing a heavy caseload (up to 4 patients a day); time management skills are essential. However, it is important to provide each patient with the time and attention necessary to meet his or her needs.
Entry level offers start at $43.00 / hour and increase based upon experience. Mileage reimbursement ($.67) included.
Internal Referral Bonus: $5,000

Job Qualifications/Experience:Current NJ RN LicenseMinimum of one-year experience in acute, subacute or home/hospice care settingExperience using an EMR, excellent critical thinking and communication skillsNJ Driver's license in good standing.
Duties & Responsibilities:
Performs Patient assessments.Accurately assesses patient's appropriateness for hospice care, learning needs and progress toward goalsDevelops the plan of care for the patient and familyEnsures appropriate utilization of services
Maintains visit productivity standardsDemonstrates knowledge of insurance reimbursementCompletes discharge assessments
Performs laptop transfer of data twice dailyCompletes admission assessment and transfers within 24 hoursCompletes re-visit documentation and transfersUses age appropriate teaching methods for patients and families
Evaluates teaching plan and revises as necessaryDocuments patient education tools usedAssumes responsibility for Case Management.
Develops/implements/evaluates plan of care based on evidence-based knowledgeRevises care plan as indicatedPrioritizes care needsDemonstrates knowledge of hospice regulations and insurance reimbursement guidelinesShows evidence of interdisciplinary communicationPerforms HHA supervisions per standardsEnsures appropriate utilization of servicesFollows scheduling processCommunicates progress toward goals with IDTAttends and participates in IDT meetings every 2 weeksEvaluates the plan of care and revises as neededMaintains safe environment for patients and co-workers

We're accredited by the Joint Commission and licensed by the New Jersey Department of Health. Atlantic Visiting Nurse - Home Health services are available to residents of Morris, Union, Essex, Sussex, Somerset and Warren counties in New Jersey.
Hospice
At Atlantic Visiting Nurse, we believe that those living with advanced illnesses should spend their final days in comfort and peace. Our compassionate, specialized care is designed to minimize patient suffering and to support families and loved ones in their roles as caregivers. As one of the first hospices in New Jersey and the United States, we have more than 40 years' experience serving private homes, nursing facilities and assisted living facilities in Essex, Morris, Somerset, and Union counties. When patients and their primary care physicians agree that curative treatment is no longer appropriate or desired, a referral to Atlantic Hospice may be appropriate. Our hospice team works with referring doctors to develop individualized care plans that manage pain, control symptoms, and ensure dignity and respect in the final days of life. We also have nurse liaisons at Morristown Medical Center, Overlook Medical Center, Newton Medical Center, Chilton Medical Center, and Hackettstown Medical Center, as well as other health care facilities in the area, to answer questions and ease the transition to end-of-life care.
