Job Description

PURPOSE OF POSITIONResponsible for nutrition assessments, education, counseling and breastfeeding classes.

RESPONSIBILITIES

• Provides participant-centered nutrition assessment, education andcounseling to WIC participants from different age groups.

• Assists in planning and implementing the Language Access and Nutrition Education component of the WIC Program.

• Acts as a member of the Interdisciplinary Team and participates in Performance Improvement activities.

1. Plans and conducts group and individual nutrition education and breastfeeding classes for WIC participants as assigned, using participant-centered counseling and facilitated group counseling skills.2. Assists in planning and implementing the program's goals of ensuring language access to all WIC participants and providing participant centered nutrition education.3. Performs complete nutrition assessment using participant-centered techniques, and ensures that all nutritional risk criteria, including High Risk and CPA-generated risk criteria are accurately identified, interpreted, and entered in WIC computer system.4. Demonstrates participant-centered skills in all interactions with WIC participants, including critical thinking, motivational interviewing, and rapport-building.5. Certifies eligible WIC applicants, using the WIC computer system according to NYSDOH regulations.6. Conducts on-site anthropometry and hematology as needed.7. Prepares High Risk care plan for High Risk participants and provides appropriate follow-up care/ nutrition counseling; documents High Risk contacts including goals set by the participant in WIC computer system.8. Refers participants to other health professionals or agencies as needed.9. Documents in WIC computer system the nutrition education provided and any referrals made.10. Prescribes, tailors and issues food packages to certified participants as defined by NYSDOH.11. Reviews and approves formula requests requiring medical documentation for participants, in consultation with the health care provider as needed.12. Conducts self-evaluations and peer reviews of QNs/CPAs as assigned,13. Complete Outbound and Inbound Verification of Certification (VOC).14. Schedule WIC participants for subsequent certification, for secondary and follow-up nutrition education contacts as required.15. Able to plan, organize, coordinate, and implement special events/nutrition promotional activities, such as Nutrition Month Celebration, Breastfeeding Week Celebration, Physical Fitness Day or FITWIC Fun Day, Farmer's Market Promotion Day, Special Food Demo Day, etc.16. Demonstrates a professional manner, warmth and respect of participants, to enhance and further the Hospital's Mission/Vision and Values.17. Attends staff education as required by the Hospital and NYS DOH.18. Participates in outreach activities, performance improvement activities or other special projects as assigned.19. Performs other functions and/or special assignments as the need arises.20. Adheres to Hospital and Departmental Policies and Procedures.

Minimum Qualifications

Qualification Requirements:

For appointment to Assignment Level I1. Baccalaureate degree from an accredited college or university, with a major in Dietetics or Foods and Nutrition in a program approved by the American Dietetic Association (ADA); or,2. Registration or eligibility for registration by the Commission on Dietetic Registration (CDR) as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.

For appointment to Assignment Level II1. Registration or eligibility for registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.

For appointment to Assignment Levels III and IV1. Registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.

Special Note for Dietitians assigned to Post-Acute Care (Long-Term Care) facility:Under the Medicare and Medicaid Programs, Reform of Requirements for Long-Term Care Facilities, as outlined in Section 483.60 of Title 42 of the Code of Federal Regulations all newly-hired Dietitians, Levels I-IV assigned to a Post-Acute Care (Long-Term Care) facility, are required to have New York State Certification, issued by the New York State Education Department (NYSED), as a Dietitian-Nutritionist at the time of hire. Dietitians, Levels I-IV hired prior to November 28, 2016, assigned to a Post-Acute Care (Long-Term Care) facility are required to obtain New York State Certification, issued by the NYSED, as a Dietitian-Nutritionist on or before November 28, 2021.
