Please attach a copy of your resume to your applicationthank you!Schedule: Full-time, daysLocation: 7700 S BROADWAY, Littleton, 80122The value youll bring to the team:Provides independent advanced practice nursing care including but not limited to completions of assessment, diagnosis, development of treatment plan including outcome identification, patient/family education, and implementation and evaluation of the treatment plan.Performs patient rounding with an interdisciplinary team and participates in case conferences.Performs comprehensive physical, psychological, social, spiritual, and cultural assessments through observation, interview, review of medical records and clinical data inclusive of imaging and lab results.Evaluates patients response and intervenes to ensure optimal patient outcomes

QualificationsThe expertise and experiences youll need to succeed:Palliative care experience desired - inpatient or outpatientBachelors degree, Nursing- RequiredMasters degree, Nursing- RequiredBasic Life Support- American Heart Association - RequiredRegistered as Advanced Practice Nurse - RequiredRegistered on the State of Colorado Advanced Practice Registry - Required
