Responsibilities:
Under the direction of the professional nurse, the Home Health Aide assists the patient in the home setting with personal care activities and incidental duties which the patient requires. The Home Health Aide assures the assessment of care is appropriate to the age of patients served: adolescents, adults and geriatrics.Essential Functions:The Home Health Aide provides personal care to adolescent, adult and geriatric patientsAssessing the patients temperature, pulse, respirations, and blood pressure to monitor his/her condition.Changing and making bed to maintain cleanlinessAssisting patients with prescribed exercisesAssisting patient with application of TED/JOBST stockings.Observing patients condition and reporting changes in condition to the Registered Nurse or Supervisor.Assisting with patient transfers, ambulation, and prescribed exercises according to established procedures.Documentation of care provided and other pertinent observationsGrand View Health's Home Care department serves Bucks and Montgomery communities. Grand View Health Home Care strives for excellence by assisting patients with their health needs in the comfort of their home. The Home Care staff is expected to commute within a 15-mile radius of Grand View Hospital.
