Job Objective:
The Transitional Housing Ambassador Program Case Manager ensures the orderly provision of Transitional Housing Ambassador Program services to all clients in residence. Follows the progress of assigned clients to ensure they are completing their goals through the recovery program. Assists with ensuring the safety of all clients while collaboration with Security, Outpatient Services and the THAP team. Coordinates the delivery and monitors all services provided internally or externally for the client. Acts as a liaison with community resources for client needs. Develops individualized treatment service plan for assigned clients. In a collaborative environment with the counselor and other HLC staff, develops appropriate addictions treatment goals. Monitors client's progress, documents progress of goals and all contract with clients. Interact with clients and provide a listening ear and respond empathetically to client needs without counseling. The Transitional Housing Ambassador Program Case Manager serves to ensure the Mission of The Salvation Army is effectively carried out.
What You Will Do:
Develops case management plan for each client who is willing to participate in the case management process. Case plan to include but not limited to the following areas:
Housing
Employment and Job Readiness
Legal Services
Personal Finance
Public Assistance
Health, including Medical and Mental Health
Tuberculosis, HIV, Communicable Disease Screening
Child Care, Prenatal Care, including immunization for children
Parenting
Transportation
Material and Financial Assistance
Enhancement of Family Function
Spiritual Concerns
Veterans' Benefits
Functions as a liaison on behalf of the client with other agencies; functions as advocate for clients of the Center as needed
Maintains records and keeps statistical reports as required by The Salvation Army and other contractual or funding entities
Assists in maintaining a structed environment for clients
Coordinates, manages, and monitors individualized service plan
Provides support, encouragement, and compassion to clients as needed
Assures clients transportation as needed and appropriate
Performs client terminations in cases of dangerous or threatening behaviors
Adheres to the policy and procedures of The Salvation Army Harbor Light Center
Serves on Safety Committee provide feedback to Transitional Housing Ambassador Program Staff
Serves on Transitional Housing Ambassador Program Committee
Coordinates, manages, and monitors client phase requirements
Conducts urine drug screens for clients according to policy and procedure
Provide support, encouragement, and compassion to client as they work toward achieving their goals. Provide guidance while expecting client self-responsibility
Coordinates daily activities of the residential ambassador
Maintains client confidentiality of all client information according to Federal, State and The Salvation Army standards, policies, and procedures
Attends multi-disciplinary staffing, departmental and staff meetings, as scheduled. Participate in planned in-service training as well as other training as directed
Maintains attendance in accordance with agency guidelines
Other tasks as assigned by leadership
Minimum Qualifications:
The requirements listed below are representative of the minimal education, experience, skills, and/or abilities required for this position.
Education
: Bachelor's degree in Human Services with a minimum of 2 years providing direct services or High School Diploma with 5 years minimum of direct service. Additional job training and certifications will be required.
Background Checks
:
Position requires a background check to be completed. Findings may disqualify an individual for this position
Experience:
Minimum of 2-5 years direct services
Certifications:
Certified by ICAADA or any other additions certifying body expected. National Certified Peer Recovery Support Specialist or Recovery Coach Certification required or must be obtained within the first 90 days of hire. Must have a valid driver's license and maintain The Salvation Army Driver's qualification standard; must complete Safe From Harm training within the first 90 days of employment.
Skills/Abilities:
Able to
speak, write and understand English in a manner sufficient for effective communication with leadership, field personnel, and clientele
Computer proficiency with Microsoft products and ability to learn electronic reporting systems
Bilingual and able to read, write and speak Spanish preferred.
Supervisory Responsibility:
No supervisory responsibilities

Work is performed in a typical office environment;
Full-time position; may require some weekend and evening work
