What You’ll Do:
Provide exceptional nursing care to patients in their homes
Collaborate with healthcare teams to create and implement care plans
Communicate effectively with patients, families, and team members
Demonstrate empathy and compassion in all patient interactions
Travel throughout the county to complete hospice visits
What You Need:
Valid RN license in California
At least one year of experience in hospice or home healthcare
Certification in hospice and palliative care (CHPN, CHPPN, CHPCA, etc.) is a plus
Proficiency in English; bilingual in Spanish or another relevant language is a plus
Strong communication skills and ability to work collaboratively
Willingness to work flexible hours, including evenings and weekends
Ability to drive to patient homes throughout the county
