What You’ll Do:Provide exceptional nursing care to patients in their homesCollaborate with healthcare teams to create and implement care plansCommunicate effectively with patients, families, and team membersDemonstrate empathy and compassion in all patient interactionsTravel throughout the county to complete hospice visits
What You Need:Valid RN license in CaliforniaAt least one year of experience in hospice or home healthcareCertification in hospice and palliative care (CHPN, CHPPN, CHPCA, etc.) is a plusProficiency in English; bilingual in Spanish or another relevant language is a plusStrong communication skills and ability to work collaborativelyWillingness to work flexible hours, including evenings and weekendsAbility to drive to patient homes throughout the county
