to hospice and home health patients, ensuring their comfort and dignity.Oversee
wound care
and perform
medication management , including pain relief and symptom control.Develop and execute
personalized care plans , coordinating with patients, families, and the care team.Maintain detailed
hospice documentation , adhering to state and Medicare regulations.Travel within Sonoma County
to provide in-home patient care.
What We’re Looking For:Registered Nurse (RN) license in California
(active or in process).Experience in
hospice or home health
preferred, but strong clinical backgrounds are welcome.Expertise in
wound care, medication management , and
care planning
is highly valued.Flexibility to
travel within Sonoma County .Open to both
full-time and part-time
roles – we’ll work with your schedule.
