Job Summary
Assists with attaining the University Medical Center goal of obtaining accurate medication histories within 24 hours of admission. This activity will consist mainly of face to face interviews with patients in the Emergency Department and with patients directly admitted to inpatient units.
Responsibilities
Interviews patients in the Emergency Department to ascertain an accurate home medication list.
Utilizes available software to collect home medication list.
In the event that the patient is unable to supply the medication list, the following may need to be contacted: Physician’s office, Mail-order Pharmacy, Retail Pharmacy, Extended Care facility, Another acute care facility
Obtains medication allergy information.
Appropriately records the information obtained either on paper or electronically.
Communicates appropriate information as necessary to other healthcare professionals.
Shift rotations may be necessary in order to cover needs.
Other duties as necessary to fulfill the needs of our patients
Required Qualifications
High School Diploma or GED
Registered as a pharmacy technician with the Commonwealth of Kentucky
PTCB national certification or NHA
Preferred Qualifications
Retail pharmacy experience
