DescriptionThe Case Manager will provide general leadership, supervision, and management over assigned service area and staff. The Case Manager will serve as a liaison between the caseworker and Leadership Team. They will also work closely with the Utilization Management Team in order to coordinate services. The Case Manager must have excellent communication and documentation skills. The Case Manager will possess an understanding of all mental health and developmental services. The Case Manager will play an active part in the interdisciplinary team.This full-time position may require more than 40 hours a week on the job. Self-monitor the use of own time and the time of co-workers if it affects this position in the areas of work hours, schedule, breaks, lunches, etc. Report any needs in this area to the supervisor and/or “chain of command”. Respond to after hour calls in accordance to procedures.RequirementsMaintain a caseload of 25 weekly billable insurance hours.Ensure Client Rights are being adhered to for all consumers.Provide designated weekly reports to supervisor.Attend weekly meetings with direct supervisor for supervisions.Participate in multidisciplinary staffing as required.Maintain caseload of assigned consumer hours.Collect data and submit for continuing service authorization, maintain up-to-date service authorization tracker to sustain 100% authorizations on all consumers within team.Input consumer demographics into OnTarget.Upload relevant documents into OnTarget.Convening the multidisciplinary team for person-centered planning and treatment plan updates.Review any revisions of the Individual Treatment Plan and monitor and document the status of the consumer’s progress and the effectiveness of the strategies and interventions outlined in the Treatment Plan as necessary.Consult with identified medical (for example, primary care and psychiatric) and non-medical (for example, DSS, school, DJJDP) providers, engaging community and natural supports, and including their input in the person-centered planning process.Review/Approve all assigned staff’s service notes to ensure they are billable and in accordance with regulatory standards.Provide weekly supervision to assigned staff and review and assess service schedules.Provide routine monitoring for services being rendered by team members for quality assurance in accordance with Medicaid Part 132 requirements.Deliver direct care services when necessary to ensure consumer continuity of care.Participate in quarterly peer-review audits.Conduct monthly outcome summaries and progress of consumer goals.Complete discharge summaries within 7 days of discharge.Other duties as deemed necessary to provide quality and continuity in services provided.Responsible for ensuring that services are provided in a manner that meets all applicable regulatory licensure and accrediting regulations.Report critical concerns and problems to Supervisor in a timely manner and actively seek Supervisor’s input and assistance in resolving the concern.Follow chain of command when encountering problems or other areas of need and correct problems requested.Solicit and cooperate with assistance from all support staff.Actively participate in an appropriate manner with the decision-making process as a “pro-active” team member and cooperate with the team’s decision, whether programmatic or clinical.Provide coverage on-site or as assigned by the needs of the agency.Attend staff meetings, planned in-service training, ongoing training, and any other regular or assigned meetings.Responsible for any equipment assigned in order to perform duties.Facilitate flow of information to all necessary staff.QualificationsBachelor’s Degree in Social Work from an accredited college/university with a combination of three (3) years documented experience in the delivery of mental health treatment to the population served, HS Diploma/GED with a minimum of 5 years documented experience in the delivery of mental health treatment to the population served, or master’s degree in Social Work from an accredited college/university with minimum three (3) years documented experience in the delivery of mental health treatment to the population served (QMHP level standards and credentialing requirements preferred).Proficient in Windows/Microsoft Office programs including Word, Excel, email.Knowledge of Electronic Health Recordkeeping.Provide current proof of applicable licensing and credentialing requirements.Bilingual (Spanish or Polish preferred).Must be 21 years of age.Successfully pass criminal records check, fingerprinting, and HCWR check as a prerequisite for employment consideration.Must successfully complete all core required trainings and updates as needed.Must have reliable transportation, maintain required level of vehicle insurance, and valid driver’s license.Knowledgeable about CARF, State, Federal, and Agency Compliance, Documentation Skills, Attention to Detail, Reporting Research Results, Thoroughness, Excellent Written and Verbal Communication, Presentation Skills, Professionalism, Objectivity, and supervisory experience.
