The HOUDINI Link Lead Navigator will provide ongoing support and/or consultation to the navigator team regarding the day-to-day operations of the program checking data entry for consistency and errors, providing suggestions and ideas to improve workflows, assisting the Project Director with onboarding tasks of new navigators and outreach to community partners & organizations. The lead navigator will also verify that program referrals meet eligibility criteria prior to enrollment and collect Patient Satisfaction Surveys and missing data for all participants at 3- and 6-month data collection timepoints. The lead navigator may facilitate occasional navigator meetings to review cases, discuss or navigate challenges, or celebrate successes. HOUDINI Link is a small yet efficient team, therefore the lead navigator may also maintain a small caseload of five participants, take on navigator responsibilities or fill in as needed when other navigators are out.Project HOUDINI LINK (Hospital Opioid Use Disorder treatment INItiation and LINKage to care) will provide medication assisted treatment (MOUD) and comprehensive psychosocial services to individuals aged 18 and older with opioid use disorder (OUD) who present to Zuckerberg San Francisco General (ZSFG). The population of focus will be hospitalized patients who are initiated on MAT for OUD, San Francisco residents and referred to diverse outpatient MOUD programs within San Francisco. Patient navigators will work closely with multi-disciplinary teams of nurses, physicians, social workers, etc. at ZSFG to link and retain enrolled patients in outpatient care using contingency management and motivational interviewing. Patient Navigators are also responsible for recruiting project participants, linking patients to primary care and mental health care as necessary, and re-engaging patients that may drop-out from MOUD. Acts as a communication liaison to various treatment providers and organizations. Facilitates patient scheduling as needed. Provides support during clinic visit as needed. Gathers and documents patient issues, barriers, or concerns through regular patient visits and perform data collection for ongoing program evaluation.The final salary and offer components are subject to additional approvals based on UC policy. Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. The mission of the Division of Substance Abuse and Addiction Medicine (DSAAM) is to improve the quality of life for our patients and the public by providing the highest quality addiction treatment, education, and research to reduce the dangers of drug abuse and its consequences. DSAAM provides multi-disciplinary consultation and treatment for patients with substance use disorders at Zuckerberg San Francisco General Hospital (SFGH) and several affiliated clinics (including OBIC Clinic). The main clinics include a licensed opioid treatment program (Ward 93/OTOP), the nation's first buprenorphine induction clinic (OBIC Clinic), mobile methadone vans, and a linkage program (HOUDINI LINK).Project HOUDINI Link staff will come from DSAAM in the UCSF Department of Psychiatry and Behavioral Sciences which has over 40 years of serving the population of focus and providing comprehensive care management to complex, underserved populations. HOUDINI Link staff also partner with the UCSF Departments of Family Medicine, Medicine, Emergency Medicine and the ZSFG inpatient Addiction Medicine Consult Service. Project HOUDINI Link staff are highly trained individuals who are state and nationally recognized leaders in the treatment of OUD. Referrals to residential treatment, primary care and treatment of mental illness are available to provide a continuum of care individualized to meet patients' needs.Required Qualifications
Bachelor's degree in related area and minimum five years of experience or equivalent experience / training.Advanced knowledge of Patient Rights & Responsibilities, Joint Commission standards, and Centers for Medicare / Medicaid regulations. Knowledge of Medical Terminology. Advanced knowledge of data collection, compilation, and analytical techniques.Advanced skills to comprehend and assess patient's grievances to quickly locate appropriate resource for assistance. Advanced knowledge of the organization and how to get issues resolved.Advanced interpersonal and customer service skills. Ability to communicate and resolve issues effectively with a diverse population of patients, staff and physicians.Advanced analytical and problem-solving skills. Ability to develop solutions and recommend changes and follow through with implementation.Advanced written and verbal communication skills.Proficiency with Windows-based software including Microsoft word, Excel, Outlook. Knowledge of computer systems and software used in functional area.Strong ability to uphold professional boundaries.Knowledge of various supportive counseling strategies (Motivational interviewing, wellness & recovery principles, harm reduction & recovery, trauma informed care, culturally responsive care, group facilitation and more).Training and experience in delivering behavioral health interventions to persons with substance use disorders as well as conducting point of care urine drug screens with fidelity to established policies & procedures.Strong skills to work as part of a team, to prioritize and handle multiple tasks simultaneously and work independently in a fast-paced environment with minimal supervision. Ability to work collaboratively with other community agencies.Ability to remain calm under pressure and apply sound judgment.Strong aptitude to provide culturally and linguistically appropriate behavioral health services to the population of focus.Preferred Qualifications
Ability to speak, write, and understand Spanish.License/Certification
