Advance Your Career at Providence HealthReporting to the Patient Care Manager or Resident Care Manager and working in collaboration with the interdisciplinary team, the Licensed Practical Nurse (LPN) is responsible for providing nursing care to an assigned group of patients/residents according to established policies and procedures and the Standards of Practice for the Licensed Practical Nurses of British Columbia. The LPN functions as an independent practitioner, while recognizing situations such as increased patient acuity or care that falls outside the scope of an LPN as appropriate for accessing the RN/RPN for collaborative support and/or clinical intervention.Your duties will include:Assists patients/residents in meeting physical, social, spiritual, and psychological needsConsistent with mission, vision, and values of Providence Health Care (PHC)Participates in care planning, assessments, interventions, and evaluationsDocuments information and advocates for patients/residentsAttends various meetings including care conferencesAssists with orientation of new staff and studentsResponds to and reports incidents following established proceduresPerforms clerical duties as requiredWhat You Bring

Education:Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experienceRegistration with the British Columbia College of Nurses and Midwives as a practicing LPN registrantCurrent basic cardiac life support certificationSkills & Abilities:Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of servicePhysical ability to carry out the duties of the positionAbility to apply decision making and analytical skillsAbility to role model conflict resolution strategiesAbility to prioritize and organize workAbility to teach and identify patient/resident learning needsWhat We Offer

Our hospice/palliative care sites include the following: PHC has a 12-bed in-patient Palliative Care Unit at St. Paul's Hospital (10D) and also operates a freestanding hospice, St. John, located on the UBC campus.Your Day to DayProvide safe and respectful nursing care, adhering to established policies, individualized care plans, and prioritiesConduct thorough assessments, monitor signs/symptoms, and communicate with the RN/RPN as neededPrepare patients for procedures, transfers, and admissions, coordinating with the interdisciplinary teamAdminister medications per protocol, monitor effects, and maintain communication with the teamUtilize evidence-based resources to stay updated on nursing practicesRespond to incidents such as codes, injuries, spills, and alarms, following proceduresAssist with staff orientation, demonstrating procedures and providing supportAddress patient/resident and family learning needs, provide educational materials, and make referralsAttend care planning meetings, provide input, and advocate for patients/residentsCollaborate with the team to develop and evaluate individualized care plansAssist with discharge planning based on patient status and external supportsOperate and clean equipment, report malfunctions, and handle hazardous materials.
