offers a wide range of services to pediatric patients seeking care from any of our 15 sub-specialties. Some of the pediatric sub-specialties we see include surgery, dermatology, endocrine/diabetes, nephrology, urology, gastroenterology, pulmonology, muscular dystrophy, allergy, solid organ transplant, weight management, and adoption medicine. Our teams practice patient and family centered care, partnering with families to ensure they are a partner in the care our patients receive.What you will do as an
RN Care Coordinator
:Coordinates care across the health care continuum within designated sub-specialty population(s)Serves as a single point of contact by facilitating and coordinating transitions in care within and between care settings, providers, and servicesCompletes pre-visit planning and post-visit wrap-up and determines vital follow-up appointments and/or treatments to optimize patient visitsFacilitates proactive disease management and preventative health care using evidence-based practiceRooms patients for the surgeon(s) as needed, complete clinic readiness including records review and provides administrative support as needed.What you will need:Current Minnesota RN licenseBLS certificationDemonstrated experience in patient advocacy and education, coaching, and proactive health care management along with EMR/digital literacyLocation:
