The Bilingual Team Nurse, LVN is responsible for providing primary care nursing at all Petaluma Health Center sites. The Bilingual Team nurse, LVN will implement plans of care, advise patients on health maintenance and prevention, maintain clinical documentation, mentor other staff, and/or provide case management per protocol under the direct supervision of a Registered Nurse or Provider.Areas of Responsibility:Patient Care & Education

Performs nurse visits in person and telehealth under the direction of Provider and per protocol (includes but not limited to): Patient education in the management of hypertension and diabetes including insulin, glucometer teaching, basic follow up wound care including suture removal, specialty injections including hormone injections, manages follow up LTBI treatment, memory evaluations using MOCA or SLUMS test, medication reviews.Performs comprehensive data gathering and advice (phone and walk-in) for adult/geriatric, young adult, pediatric, infant under the direction of licensed clinical staff.Informs and educates patients on the significance of abnormal lab results relative to chronic disease and/or Public Health tracked illnesses in accordance with PHC approved educational materials as per protocol.Collaborates with provider to identify patient priorities to increase patient chronic disease understanding.Processes medication refills in accordance with protocol.Performs annual competencies to demonstrate proficiency in patient care and also evaluates colleagues to validate proficiency.Performs medication reviews and adjustments per protocol and with support of Provider.Effectively utilizes PHC approved educational materials to provide patient education for medications, wellness, disease management, procedures and processes under the direction of licensed clinical staff.Provides basic emergency care under the direction of a RN or Provider.Manages inventory and waste of controlled medications.Documentation & Work Environment

Documents patient care provided in visit in a timely manner in the electronic medical record.Documents triage assessments appropriately in the electronic medical record.Documents and reports malfunction of PHC equipment.Maintains a clean, orderly and functional work environment.Promotes continuous improvement of workplace safety and environmental practices.Case Management

Performs indirect case management (includes, but not limited to): population health, missed follow up care, and new patient intakes.Provides direct case management for uncontrolled chronic disease with Provider direction.Training

Assists in staff trainings.Provides guidance for MA's doing non-appointment work.Mentors and orients new staff, students and other team members.Education/Experience

Equivalent to an LVN degree from an accredited school of licensed vocational nursing.One to two years of increasingly responsible nursing experience including experience in community nursing preferred.Licenses and Certifications

Possession of an appropriate, valid California Licensed Vocational Nursing License.Possession of, or ability to obtain, an appropriate valid California driver's license.Possession of, or ability to obtain, an appropriate valid BLS certificate.Candidate must be able to successfully meet PHC's credentialing and privileging requirements.Language Skills

Bilingual in English and Spanish, both written and verbal required. Assessed by language proficiency test as Advanced or Intermediate.
