Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Completes assessments, intake evaluations, outpatient assessments, person-centered treatment plans, family/individual therapy (in various settings office and in field).Assists in program development and implementation.Provides the professional expertise necessary to make determinations regarding patient eligibility.Makes recommendations related to the care needs regarding the clients’ health and safety.Participates in child and adult case coordination meetings and assists participants as needed with accessing resources, as well as obtaining consent and authorization forms.Identifies and responds to high-risk participant encounters with elevated safety concerns, following company protocols.Screens & assesses for acuity levels. Determines appropriate programs based on client needs & risk assessments.Participates in regularly scheduled meetings with supervisors, monthly team meetings, and teaming meetings with collaborative.Maintains the highest standards of confidentiality as described in HIPAA Privacy & Security policies and ensures compliance with annual training requirements.Completes paneling to all insurances needed for Hillcrest consumers.REQUIRED EDUCATION AND EXPERIENCE
Master’s degree in social work, psychology, or related discipline is required.Must have one of the following District of Columbia certifications: LICSW, LPC, LGPC, or LGSW.
