The Salvation Army is seeking applicants for a full-time case manager. This position is part of the Joseph McFee Center (JMC), a recovery-focused transitional housing program for San Franciscans exiting residential treatment. The JMC provides participants the opportunity to build on progress made in early recovery and holistically work toward a healthy, independent, and prosperous life. Under the supervision of the Program Manager, the Case Manager’s main goal is to address the needs of JMC participants and cultivate a transformational community based on the program's principles of recovery, accountability, and service.DUTIES AND RESPONSIBILITIES:Manage an assigned caseload of up to 25 program participantsAssessment and Intake: Conduct interviews and administer assessments to understand the client’s needs, situations, and potential risks. Gather relevant information about the client’s background, family dynamics, living conditions, or any other relevant factorsBased on the assessment, create an Individual Transition Plan that outlines specific goals and objectives as well as interventions that address the client’s unique needsEstablish and maintain relationships with partner organizations and referring agenciesMonitor client progress and reassess their needs to ensure that the goal plan remains relevant and effectiveEducate and empower clients by equipping them with the knowledge and skills needed to improve their situation and become more self-sufficientRespond to and manage crisis situations clients may encounterMaintain appropriate, professional, and ethical boundaries with staff and clientsBe punctual, flexible, and reliableMaintain accurate client records on The Salvation Army Social Services DatabaseAttend all required staff, Corps, and Divisional meetings and trainingFacilitate small group instruction as indicated on the program scheduleAttend community-based meetings as neededIdentify appropriate resources, programs, and services that can benefit the client and make referrals to these servicesCoordinate with various service providers to ensure the holistic needs of clients are metOther reasonably related duties as assigned by the Program Director or Program ManagerAssist in the planning, preparation, and execution of special functions such as graduations, holiday celebrations, group activities, or special classesDriving clients to appointments or servicesOther related duties as requiredEDUCATION AND EXPERIENCE:High School Diploma (required)CA Driver's License (required)Experience working with adults in recovery, reentry, or disadvantaged populations (2 years required)Community Health Worker Certificate or higher, and 1 year of experience providing case management services to adults in recovery from addictionExperience and ability to work successfully with issues of substance abuse, mental illness, and other potential barriers to self-sufficiencySKILLS AND ABILITIES:Strong verbal and written communication skillsAbility to express empathy and foster a collaborative alliance with clientsExcellent organizational skills and ability to work with little direct supervisionStrong electronic communication skillsProficient in Microsoft Word & ExcelKnowledge of San Francisco social services, government resources, community resources, healthcare services, mental health support, and housing servicesKnowledge of co-occurring disorders and trauma-informed environmentCulturally competent and able to work with a highly diverse populationCOMPENSATION:$28 - $31.25 HourlyPHYSICAL REQUIREMENTS:Ability to lift up to 25 lbs.
