Job Title: Navigation Center Advanced Practice PractitionerDepartment: Medical AdministrationReports to: Medical DirectorFLSA Status: ExemptWage Range that the Company Expects to Pay: $5,000.00 - $5,736.82 bi-weeklySUMMARY:The Family Advanced Practice Practitioner reports to the Director of Family Medicine regarding administrative, provider relations, and other day-to-day guidance. Assigned to supervising physician under the California Medical Board for medical consults and abides by appropriate clinical protocols. Facilitates clinical issues between clinicians and provides written and verbal guidance between clinics and the Navigation Center to meet patient needs. Provides patient care via Telemedicine, and phone consults and addresses medication refill requests as needed.ESSENTIAL DUTIES AND RESPONSIBILITIES:Ensures call center staff operates according to published clinical guidelines. Ensures adherence to existing Policies and Procedures to facilitate seamless patient care.Serves as liaison to physicians and other departments on referrals and walk-ins.A key role of this clinician is to triage and address patients' concerns that avoid unnecessary Emergency Room visits and to facilitate seamless patient resolution for valid patient concerns.Triages, diagnoses, treats, and/or refers patients assigned on an outpatient or telemedicine basis. Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assess medical conditions consulting with supervising physician as needed.Maintains a problem-oriented health record for each patient seen in accordance with the Center’s established Health Information Management policies and procedures to include providing appropriate and legible documentation.Completes thorough documentation of each patient’s visit in the electronic health record and ensures encounters are closed within 24 hours in accordance with the Center’s established Health Information Management policies and procedures.Assures continuity of care by providing medical services on a timely basis as per Clinic schedule within the Center’s scope of practice and the Nurse Practitioner’s training.As needed will review and report lab results, address medication refill requests from patient and pharmacies, additionally release and review imaging reports for patients that may be under their or other clinicians care in a timely manner.Provides excellent internal/external customer service via telephone consult, Telemedicine, face-to-face contact to assist patients with their health care needs.Will provide clinical oversight over clinical support staff engaged in activities and procedures, as appropriate.Follows established departmental and clinic policies, procedures, and objectives; continuous quality improvements objectives and safety, environmental, and/or infection control standards.In the absence of established policies, will work with other departments to develop such policies, procedures, standing orders, and overall objectives for the clinical arm of the Navigation Center function.Participates in specified health promotion, education and/or prevention programs as needed i.e. diabetes collaborative/health fairs etc.; attends and participates in clinic meetings / departmental meetings and other clinic functions as required by the Center.Performs other related duties as assigned.JOB QUALIFICATIONS:Master’s Degree from an accredited university in a Nurse Practitioner Program or Physician's Assistant Program.Previous experience as Family Nurse Practitioner/Physician’s Assistant highly preferred.Must have a current unrestricted license to practice as a Nurse Practitioner from the CA Board of Nursing or as a Physician Assistant from the CA Medical Board.Annual continuing medical education as required by board specialty.Must be registered and have current DEA.Experience with Telemedicine highly preferred.Bilingual in English and Spanish highly preferred.Certified in Basic Life Support (BLS), American Heart Association.Scheduled hours and/or work locations are subject to change.SUPERVISORY RESPONSIBILITIES:May supervise students at the direction of the Medical Director.QUALIFICATIONS:Active California Medical Physician Assistant (PA-C) or Certified California Registered Nurse Practitioner (CRNP) License.Active DEA License.Master’s degree in Physician Assistant studies or Nurse Practitioner program from an accredited college/university.1 year postgraduate experience working in a clinical outpatient setting.Preferred: Psychiatric-Mental Health NP certification for NP’s and Certificate of Added Qualification (CAQ) for PA’s.Motivated to develop strong clinical skills in psychiatric medicine.Comprehensive knowledge of DSM-V diagnoses and diagnostic criteria.Basic computer and internet skills.LANGUAGE SKILLS:Ability to read, analyze, and interpret the most complex documents. Ability to respond effectively to the most sensitive inquiries or complaints. Bilingual (English and Spanish) ability to speak, read, and write Spanish desirable.MATHEMATICAL SKILLS:Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to interpret bar graphs.REASONING ABILITY:Authoritative knowledge of the principles, practices, and techniques of psychiatric and family-centered medicine. Ability to apply principles of logical or scientific thinking to a wide range of intellectual and practical problems.CERTIFICATES, LICENSES, REGISTRATIONS:Possession of current unrestricted and valid License - NP/PA or equivalent from Medical Board of California. Possession of current unrestricted and valid DEA license; and Basic Life Support (BLS-C) card.Annual health examination; annual TB clearance, and up-to-date immunizations for health care workers at time of hire and annually thereafter.PHYSICAL DEMANDS:The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
