$77,265.00 (Min.) - $100,445.00 (Mid.) - $123,625.00 (Max.)

The Practice Transformation Project Manager II plays a key delivery role in helping physicians transform their practice and improve clinical outcomes and patient care. This position will work independently and collaboratively with practices and the Practice Transformation team to achieve program goals.

Duties

Develop and implement practice work plans to achieve improvements in health care delivery and outcomes.

Provide tailored technical assistance to practices, including provider outreach, practice facilitation, and change management.

Track project goals and clinical quality measures in electronic health records (EHR) systems and report monthly results.

Conduct regular practice assessments using L.A. Care program tools.

Work with program leads to implement project strategies and ensure timely completion of deliverables.

Perform other duties as assigned.

Education RequiredBachelor's Degree in Health Administration or Healthcare Related Field. Equivalent education and/or experience may be considered.

Education PreferredMaster's Degree

ExperienceRequired:

At least 3-5 years of experience in the health care industry with implementing quality improvement methodologies.

Experience with Electronic Health Record software implementations and quality improvement programs.

Experience working with medical practices and care teams.

SkillsRequired:

Knowledge of workflow processes in clinics and quality improvement reporting.

Strong interpersonal skills, project management, and communication skills.

Able to work independently and demonstrate initiative within a healthcare team environment.
