Job SummaryThe Practice Transformation Project Manager II plays a key delivery role in helping physicians transform their practice and improve clinical outcomes and patient care. This position will work independently in carrying out the goals of the program/initiative(s), and collaboratively with practices, program principals, and the Practice Transformation team. This position uses their knowledge to assist practices in achieving the program/initiative’s goals and improving clinical outcomes.

Duties

Work together with the practice(s) to develop and implement the practice work plan in support of the program/projects and/or initiative’s goals designed to achieve improvements in health care delivery and health outcomes for patients.

Provide technical assistance to practices tailored to meet their needs, which may include supporting provider outreach and education, practice readiness, practice facilitation, health information technology (“HIT”) optimization, practice workflow redesign, change management, and strategic planning.

Track project goals, clinical quality measures, and other program data in the practices’ electronic health records (“EHR”) system, and report data results monthly to the Practice Transformation team.

Conduct regular practice assessments using L.A. Care program tool(s)/approaches.

Work with L.A. Care, program/initiatives leads, and practices to implement project strategy, help with intervention design, and ensure deliverables are completed on time, within budget, and with high quality.

Perform other duties as assigned.

Education RequiredBachelor's Degree in Health Administration or a Healthcare Related Field. In lieu of degree, equivalent education and/or experience may be considered.

Education PreferredMaster's Degree

ExperienceRequired:At least 3-5 years of experience in the health care industry with implementing Plan-Do-Study-Act (“PDSA”) or similar quality improvement methodologies in a primary care, behavioral health, or human services setting.

Experience with Electronic Health Record software implementations and meaningful use experience and/or experience in quality improvement programs and workflow processes in clinical settings.

Experience in working at medical practices with providers and staff/care teams.

SkillsRequired:Knowledge of workflow processes in clinics/practices and quality improvement reporting.

Strong interpersonal skills in the healthcare arena.

Must have implemented a Plan-Do-Study-Act (“PDSA”) or similar quality improvement methodologies using a data-driven methodology.

Strong project management, communication, relationship management skills, and health I.T.

Excellent communication skills.

Able to work independently and regularly demonstrate initiative.

Able to work effectively in a healthcare team environment and manage issue resolution effectively.

Ability to work and resolve interpersonal conflict.
