The Hospital Liaison will coordinate care, advocate and facilitate continued treatment to adults and youth while hospitalized at psychiatric hospitals. The Hospital Liaison assesses treatment needs for aftercare, advocates on behalf of clients to facilitate collaborative discharge planning between the hospital social worker and the agency-assigned primary clinician. Initial rapport building and engagement are critical components of successful aftercare treatment. This position is full time. Salary based on experience starting at $45,020 annually.

Essential Duties:

Completes all required trainingsFacilitates the initial linkage meeting between the client and LBSServes as a liaison between hospital, physicians and aftercare providers for needed referralsCoordinates care to address physical, behavioral, SUD and other identified needsActively communicates with agency intake coordinator or assigned primary clinician regarding new admission information, discharge planning, projected changes and aftercare recommendationsVisit youth and adults while inpatient to provide appropriate education to providers, patients and families that are potential LBS admission.When appropriate, communicates with family members/natural supports to encourage client engagementProvides support and education to family and natural supports.Provide effective psychological interventions with clients to assist them in making optimal aftercare decisionsHas knowledge of and provides community resources to inpatient individuals and their familyMaintains statistical data regarding inpatient days and recidivismCompletes necessary documentation in EHR for existing and potential LBS clients in a timely mannerPractices person-centered planning principles and participates in planning meetings when requestedDemonstrates appropriate crisis intervention techniques when indicated.Upon hospital discharge through aftercare, visit and closely monitors individuals for suicide riskAssists clients with community referrals and maintain contact from hospital discharge to aftercare appt.Coordinate services for substance use, co-occurring, physical health and other treatment servicesWhen necessary, assist client with DWIHN or other provider screeningsPerforms other duties as required to meet program goals and objectives.This position requires a great driving record.EDUCATION AND EXPERIENCE

Bachelor’s degree in human service field and 2 years exp. with adult with SMI/SUD adults and SED youthCredentialing, Licensure

C omplete required credential processPossession of a valid Michigan Driver’s License and able to pass background checksSKILLS AND ABILITIES

Excellent verbal and written communication skills, with ability to establish professional working relationshipsAppropriately articulates the view of the agency when networking in the community.
