DescriptionThe certified counselor provides support and builds relationships with patients who have a chemical dependency diagnosis to ensure a successful transition from home or hospital setting to the Chemical Dependency unit. Support includes brief assessment of status of chemical dependency issue, assessment of readiness for change, identification of coverage for services, referral to appropriate services and follow-up to ensure services are received.Providence caregivers are not simply valued - they're invaluable.
