DescriptionProvides support to and build relationship with patients who have a chemical dependency diagnosis to ensure a successful transition from ED/hospital to the community and to reduce re-admission to the ED/Hospital. Support includes brief assessment of status of chemical dependency issues, assessment of readiness for change, identification of coverage for services, referral to appropriate services and follow-up to ensure services are received. Serves as a link between organizations serving persons with chemical dependency needs and the Medical Center to improve coordination of services.Providence caregivers are not simply valued - they're invaluable.
