By bringing together people from various disciplines and ideas from different industries, we’re seeking to address the complex challenges within our healthcare system and leaning forward into a new era of healthcare.Under the general direction of the Program Manager, in consultation with the Senior Director and Medical Director, the Business Analyst for Care Continuum in Population Health Management (PHM) will provide project support for system-wide initiatives providing care continuum interventions for Mass General Brigham patients that are discharged. As an integral member of the Population Health Management Team, the Business Analyst is responsible for analyzing, supporting, implementing and monitoring high impact projects for the care continuum programs. This is accomplished through a collaborative process working with colleagues in the acute and SNF facilities and primary care interdisciplinary teams.One of the key programs the Business Analyst will be assigned to support is the TCM (Transitional Care Management) service. This service supports patient transitions from inpatient to Skilled Nursing Facilities and to home. The goal of the service is to enhance and coordinate the delivery of patient care services along the continuum of health care and across multi-disciplinary sectors, addressing their functioning and well-being to prevent complications and avoid hospitalizations. Other initiatives include the SNF Collaborative, developing alternative pathways, and the MSSP ACO SNF 3-day waiver.The Business Analyst is also expected to assist with initiatives beyond Care Continuum based on system priorities.Principal Duties And Responsibilities

Provide Analytics and Visualization for Care Continuum and other areas

Produce routine standard reports and contract deliverables, review to ensure data accuracy and troubleshoot, as appropriate.Complete quantitative and qualitative analyses to support the objectives of assigned projects. This includes creating charts, graphs, and transforming information gathered into a format that is clear, accurate, and useful for facilitating decision-making.Support the development of key program questions to frame analytics; ensure data requested and analyses conducted answer the critical questions such as key program performance drivers and program expansion opportunities.Assist with ongoing efforts to track, report, and evaluate care continuum efforts using multiple data sources including claims and non-claims-based data.Work to improve visualization of current reports using existing tools and new tools as they become available.Project Management Support

Develop and manage all aspects of projects including scope, requirements, project plans, risk identification and mitigation plans, communication, and issue resolution.Assist with presentations and updates of findings, and make recommendations to internal and external stakeholders/leaders as needed for assigned projects.Develop project related deliverables with minimal supervision including project plans, PowerPoint presentations, status reports, meeting minutes, issue/risk logs, and meeting facilitation documents.Present updates, findings, reports, and recommendations to internal and external stakeholders/leaders as needed for assigned projects.Conduct best-practice research and ad-hoc analysis for quality improvement initiatives.Develop methodologies for evaluating and tracking success.Manage the coordination of various workgroups, committee meetings, and other best practice sharing events.Special Projects as Assigned

Establish contacts and strong working relationships with clinical and administrative leaders across Mass General Brigham entities.Participate in central and regional stakeholder, and external meetings as needed, as related to the SNF Collaborative, SNF Transitional Care Management, and MSSP ACO (3-day waiver).Minimum Qualifications

Bachelor’s degree with at least 3 years’ experience in a healthcare setting.Strong organizational and project management experience required.Strong PC / Microsoft application skills including Word, Advanced Excel, and PowerPoint.Skills/Abilities/Competencies

A hybrid work model with a mix of onsite and remote work. At least 2 days a month on-site at Assembly Row in Somerville MA. Potential for additional days meeting with practices and MGB staff. Requires travel locally to other Mass General Brigham sites. Reliable access to transportation to travel between sites for business purposes is required.The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.
