Position Summary

The Clinical Nurse Navigator/Coordinator functions as a member of the multidisciplinary team who has clinical nursing expertise in an assigned area. They focus on the clinical and educational aspects of care, acting as a clinical resource, advocate, and educator. They are present in the clinic and serve as a vital member of the care team.

Essential Duties and Responsibilities

Demonstrate a strong commitment to patient-centered care within an interdisciplinary environment.

Follow up with patients on testing and procedure results in coordination with providers.

Prepare clinic prep notes for new patients and returning patients, which includes scanning notes/imaging, contacting referring physicians or imaging groups, and contacting patients on any issues or items needed for visit.

Assist with coordination of follow-up visits, orders, and referrals.

Initiate/Participate in quality improvement, patient safety, and patient experience initiatives.

Participate in training and education of support staff.

Minimum Qualifications

Education

An Associate's degree or higher in Nursing, or an equivalent combination of training and experience.

Licensure

A current and valid D.C. license as a Registered Nurse or evidence of application for licensure in D.C.

Experience

A minimum of 2 years of experience as a Registered Nurse, OB/GYN experience preferred.

Physical Requirements

Sit, walk, reach, bend, or twist for long periods of time in a clinical setting.

Must be able to lift, carry, push, or pull up to 100 lbs. as part of the role.

Regularly exposed to healthcare settings that may require personal protective equipment.

Requires manual dexterity to operate a computer keyboard, calculator, copier machine, and other equipment.
