Homeless Support Services Coordinator, Street to Home Program

The Homeless Support Services (HSS) team, operating the Street to Home Program, provides coordination of services and direct assistance to unsheltered individuals and sheltered individuals with a history of homelessness transition to a permanent supported housing environment with the goal to secure and maintain long term housing stability. The Homeless Support Services Coordinator (HSSC) supports the mission of The Coordinating Center by providing person centered services and service coordination within the parameters of the contract and Housing First principles. The HSSC is responsible for complying with the HUD and Montgomery County Street to Home program requirements, integrating technology to maintain database(s) integrity, report writing and maintenance of documentation. The HSSC provides ongoing outreach to program recipients, works interdependently with the Coordinated Entry System (CES), outreach workers, housing locators and designated medical clinic and collaborates with community agencies to engage recipients in activities which move them from unsheltered homelessness to being stably housed. This is a non-supervisory position, reporting to the Program Director, Homeless Support Services.This position requires a commitment to and culturally competent approach to Diversity, Equity, Inclusion and Belonging (DEI&B). We believe we are stronger together.Completes certification in Critical Time Intervention (CTI). Training to be provided during initial orientation.Implements the CTI service model and activities as a time-limited evidence-based practice to support individuals transitioning out of homelessness and into safe permanent housing options.Works to help clients achieve community integration and continuity of care through facilitating community supports and connections during their initial steps and ongoing work toward successful permanent housing residency. This includes homeless populations who are veterans, people with behavioral health challenges, and especially long-term chronic homeless populations.Work intensively in one-on-one relationships with individuals engaged in Street to Home program to provide support, engagement, and referral through Housing First practices and methodology.Becomes thoroughly familiar with the housing-focused street outreach practices guiding the work of Bethesda Cares, The Center’s Street to Home program partner.Completes required and as needed face-to-face contacts based on acuity and specific program enrollment. Services are to be provided primarily in the community and outdoor spaces where unhoused individuals may be residing.This position will work collaboratively with community outreach staff with a focus on successful and rapid transition to permanent supported housing.Obtains identification documents, housing, medical and other documentation to assist recipient with completing housing applications, enrollment and obtaining of federal, state, and local benefits, completion of program application, reapplication, and redetermination process.Secures information such as medical, psychological, and social factors to jointly develop an individualized, recovery based, goal plan consisting of time specific goals and objectives (SMART). Goals/objectives to be based on individual needs, desires, strengths, resources, and limitations, addressing the complex strengths and challenges of all individuals served.Obtains documentation of disability in accordance with County and State Assistance in Community Integration (ACIS) pilot project.Provides ongoing supports for successful transition, using CTI and, including but not limited to good tenant training, skills coaching.Ensures all recipients apply to receive Housing and Urban Development Housing Choice Voucher and Public Housing benefits.Establishes strong working relationships with property owners and managers, advocates on behalf of individuals served.Accesses and records client, residential and community resource information and care coordination activity in various databases.Meets The Center’s and the Street to Home program’s contractual billing and documentation guidelines.Qualifications:Bachelor’s degree in social work or other human services field and two years related experience.Individuals with an associate degree in human/social services and a minimum of two years field experience working with unhoused individuals or in a related human services capacity require approval by Montgomery County Services to End and Prevent Homelessness.Ability to travel throughout Montgomery County and conduct home and/or community visits in person required.Working knowledge of social service and individual support services, Medicaid services, State and Montgomery County community resources, preferred.Experience with substance use, mental health, chronic health conditions and social determinants of health, preferred.Knowledge of HUD, property owner and tenant rights and responsibilities, preferred.
