View Point Health is a community behavioral health center utilizing a team of psychiatrists, nurses, licensed counselors and social workers, and other professionals to provide services to individuals who need treatment and support to cope with mental illness, substance abuse, and intellectual and developmental disabilities. View Point Health serves uninsured, underinsured, low-income Medicaid, Medicare, war veterans and some private insurance across multiple locations with a full continuum of behavioral health services and support. The services provided by View Point Health contribute to the reduction in homelessness, reduction in crime and subsequent incarcerations, higher graduation rates, lower dropout rates, decreased ER visits and decreased inpatient admissions.Assertive Community Treatment (ACT) team provides intensive intervention, treatment, rehabilitation, and support services to individuals with severe, persistent, and cute mental illnesses or addictive diseases. Participating in client-centered comprehensive assessments of psychiatric history, mental status, diagnosis, and physical health, education and employment and social development. Participating in the development of treatment and discharge plans based on observation of individual strengths, needs, abilities, and preferences. Provides effective follow-up and referral services that facilitate individual linkages, communication, and continuity of services. Facilitates individual-family interactions, keeping supervisor and other treatment team members informed of significant developments. Complete comprehensive nursing assessments and provide links to medical care.Under general supervision, learns and assesses client health problems and needs. Develops and implements nursing care plans and maintains medical records. Administers nursing care to ill, injured, convalescent, or disabled clients.
