You will:Lead and manage multiple clinical programsDrive the execution of population health management initiativesEnsure alignment of clinical services with cost-effectiveness and quality careBuild and foster strong relationships with internal and external stakeholdersDirect and mentor a dedicated team of clinical professionalsSalary Range: $112,900- $256,900/annuallyQualifications

We’re seeking a dedicated and visionary leader with:A Registered Nurse License, Advanced Practice Provider (Nurse Practitioner or Physicians' Assistant) or equivalentThree to five years of clinical experience (RN, APP, etc.)Three to five years of leadership experience in healthcare, with a focus on clinical servicesMaster’s degree in a health-care related field, highly desiredExperience in developing and leading Population Health Management strategies within a health system or managed care organization, preferredStrong communication skills across all levels of staffProven leadership abilities with a track record of implementing innovative programs that support strategic initiativesIntegrity, initiative, and the ability to manage details with a commitment to high ethical standardsExceptional interpersonal, written, and verbal communication skillsProficiency in Microsoft Office applications (Word, Excel, PowerPoint)
