To provide specialized services to young adults and adolescents who are or at-risk of becoming chemically dependent and/or their families. Provide and coordinate substance use assessment, education, training, and placement services.Schedule:

Conduct initial substance use assessment services for adolescents and young adults.Provide individual, group, and family substance use treatment and counseling.Coordinates referrals to other substance use treatment programs or facilities in the community/state, i.e. medically assisted treatment or inpatient.Provide chemical dependency consultation, education and training to team personnel.Develop and implement person centered recovery plans and document services consistent with Thrive standards.Coordinate discharge planning, follow up and aftercare.Monitor signs and symptoms of psychiatric functioning; medication regimen, and substance use.Assist in community networking and education.Provide education, training and modeling in basic daily living skills, social or parenting skills.Teach clients effective techniques for managing their medication as prescribed.Coordinate services within Thrive as needed.Services provided in the office, home, community, schools, etc.Carry and enliven a positive Thrive image consistent with the Thrive's Guiding Principles and professional code of ethics.Qualifications

Education, Experience and Competencies:

Bachelor Degree in behavioral health field or equivalent combination of education and experience required.Two (2) years of relevant substance abuse experience with adolescents and young adults.Certifications, Licenses Requirements:

CDP or working toward credentialing. LCDP preferred.Valid driver's license and automobile insurance which meets Rhode Island minimum standards required, or the ability to obtain necessary transportation in order to perform the responsibilities/tasks of the job.Physical Demands
