The program provides a unique opportunity for you to work with a team of PACE professionals committed to your health and well-being.GeneralEstablished team of Advanced PractitionersEstablished practice serving the Siouxland communityOpportunity to practice outpatient medicine in an established, well-known clinic and programExperienced staff, both clinical and non-clinical, who know the patients and the communityFull-time position, including dedicated administrative time, with flexible scheduling to ensure sufficient patient access24/7 hospitalist program is responsible for adult inpatientsFully integrated system with clinics and hospitals on EPIC EMR to improve care coordinationResponsibilities:
Shared call amongst the providers. Currently 1:4, clinic only Triaged through a call serviceAbility to work as a team member to improve the health of the people and communities we serveSpecialty skills: Geriatric care preferred, basic procedures like wart removal, knee injections, immunizations, pelvic exams, sutures, wound care, medication management, skin excisions, lacerationsEvaluates, interprets, and coordinates information to make decisions and recommendations regarding patient carePatient-focused, friendly, and willing to collaborate in a team environmentExcellent communication (written and verbal), analytical and problem-solving skillsQualifications:
