Position Details:
In this role, you will be responsible for conducting patient assessments, developing and implementing treatment plans, and providing education and support to patients and their families. You will also collaborate with other healthcare professionals to ensure a comprehensive and coordinated approach to patient care.

ResponsibilitiesConduct comprehensive patient assessments, including medical history, physical examinations, and diagnostic testingDevelop and implement individualized treatment plans based on patient needs and medical guidelinesMonitor and evaluate patient progress, adjusting treatment plans as necessaryProvide patient education, counseling, and support on disease management, medication adherence, and lifestyle modificationsCollaborate with physicians, nurses, and other healthcare professionals to ensure coordinated and efficient patient careDocument patient encounters, including assessments, interventions, and outcomes, using electronic medical record systemsRequirements:

$90,000.00 - $120,000.00 per year
