PeaceHealth is seeking a RN Home Health for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $46.71 - $72.98. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.This position is represented by a collective bargaining agreement. There may be more than one opening on this posting.Job SummaryResponsible for providing direct skilled nursing care and coordination/management of patients admitted to home care/clinic services using considerable judgment and skill in making patient care decisions. Utilizes the professional healthcare process of assessment, problem identification/planning/intervention and clinical/goal outcome evaluation.Essential Functions* Perform initial evaluation of patient/home and apply admission criteria to determine if patient is safe candidate for home health/hospice services.* Provide skilled nursing care to patients and skilled education/instruction to patients and caregivers according to the home health/hospice plan of treatment.* Performs physical assessments, develops, coordinate, modifies and manages written individualized plan of care, as ordered by the primary physician, to meet patient's long and short-term medical goals and appropriate/timely home health outcomes.* Develops and clinically manages the Home Health Aide's care plan according to the patient's evolving personal are needs.* Coordinates care, collaborates and communicates with other members of the interdisciplinary team to assure appropriate and smooth flow of care/services through the care continuum.* Performs discharge planning appropriately towards patient specific goals/outcomes and evaluate interventions and outcomes of patient care by reviewing long and short-term goals established in the plan of care.* Provide accurate, complete and timely clinical documentation in compliance with regulatory, quality and Agency guidelines.* Assist with the plan and implementation of family Bereavement services for the following patient death.* Performs other duties as assigned.QualificationsEducation* Accredited School of Nursing Required: Nursing and* Bachelor's Degree Preferred: Bachelor of Science in Nursing.Experience* 2 years Required: Acute clinical or home health/hospice nursing experience.* 1 year Required: Home health experience preferred or will orient to point of competency.* Required: Demonstrated knowledge of and ability to apply age specific principles of growth and development and life stages to meet each patient's needs.Credentials* Required: Registered Nurse in state of practice.* Required: Driver's License and proof of auto insurance.* Required Upon Hire: Basic Life Support.Skills* Basic computer skills including MS Office applications (Required)* Well organized, flexible and able to function independently (Required)* Cooperative working skills, leadership skills, and interpersonal skills and ability to work effectively with a wide variety of patients, families and staff (Required)Department / Location Specific NotesPHOR and SIUSLAW:* RN Home Health job classification in ONA bargaining unit.SEAR:* Minimum two years nursing experience required. One-year Home Health experience preferred or will orient to the point of competency.* Nurses may, at the Medical Center's option, be required to float as needed to units* May be required to float to PHMG Clinics to provide coverage as neededHome and Community:* Will accept valid driver's license in US or Canada and proof of auto insurance.Working ConditionsLifting* Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH).* Fine motor skills to be able to grasp and control medical equipment and perform precise procedures.* Push/Pull: Frequently up to 45 lbs. force (i.e., WOW, medical carts).* Perform hands on CPR at least 20 mins using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA) Frequency should be seldom or occasionally.* Ability to move around area with frequent sitting.* Bending/ stooping/ squatting/ reaching/ kneeling frequently.* Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently.Environmental Conditions* Exposure to biohazard, body fluids and airborne particles.* Must be able to complete tasks in a noisy environment.Mental/Visual* Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).* Ability to communicate and exchange accurate information.PeaceHealth is committed to the overall wellbeing of our caregivers: physical, emotional, financial, social, and spiritual.
