PURPOSE OF YOUR JOB DESCRIPTION:The purpose is to support the physician practices department and to provide coverage when the providers are out of the office.
DELEGATION OF AUTHORITY:They will report to the providers they work with and will utilize our nurses and MA's in the offices as well.

PERFORMANCE REQUIREMENT / DUTIES AND RESPONSIBILITIES:Diagnose and Treat acute and chronic IllnessPerform age-appropriate history and physicalOrder and interpret diagnostic and therapeutic tests relative to patient's age-specific needsPrescribe appropriate pharmacologic and non-pharmacologic treatments to patient's age specific needMonitor the effectiveness of treatmentsProvide education of acute and chronic medical conditionsManaging Chronic Medical ConditionsReconciling medication listFacilitating appointments to specialistProviding thorough exams and evaluations of patient's health statusFacilitate appropriate chronic care management (i.e. Diabetic eye exams, foot exams and nutritional counseling)Preventive Medicine:Keeping track of and referring patient to appropriate preventive medical screenings (e.g. Mammograms and Colonoscopies)Counseling and reinforcing patients in healthy life style habits
EDUCATION REQUIREMENTS:* Primary Care NP certified in Massachusetts
EXPERIENCE:3 years experience in a Primary Care settingProficient Computer Skills (MS Word)EMR experience requiredAbility to document in electronic health record completely and accuratelyKnowledge of clinical reminders and how to use them appropriatelyGood communication skills, both spoken and writtenWilling and able to work in a team environmentAble to change with the ever evolving healthcare fieldMaster Degree, ACLS, and PALS preferred
