If this sounds like something you want in on and can contribute to, please review the formal qualifications and duties below:Duties and ResponsibilitiesTrain new Case Managers on creating Housing Plans and other Case Management tasksSupport new Case Managers with difficult to engage clientsComplete special projects as assignedPotentially run team meetings or represent the program at community meetingsServe a caseload of approximately 25 clients, providing active case management for all assigned cases, including meeting weekly with each person individuallyAct as the lead for client’s housing-related case management, maintaining an active housing plan for each client on your caseload, based in creative solutions to homelessnessMaintain a current and thorough knowledge of community resources and utilize them to provide comprehensive, wrap-around services to clients, navigating them through the best options for their unique situationsWork collaboratively with clients to address barriers to housing, including linkages to income/benefits, mental health services, physical health services, etc.Develop effective, trusting relationships with clients and use a client-centered approach that includes motivational interviewing, harm reduction, and trauma-informed careAdvocate for your client’s needs in the shelter and when interfacing with other agencies or service providersFacilitate intakes for individuals who are referred into interim housingCreate a supportive environment and ensure clients’ progress, utilizing a strengths-based approachFacilitate wellness, enrichment, and life skills groupsProvide crisis intervention when neededMaintain client confidentiality at all times while following agency, state and HIPAA regulationsCreate and maintain accurate documentation of client information (intake, referrals, progress notes and service delivery) in HMIS database and in accordance with program funder requirementsParticipate in case conferences, individual supervision, and other staff meetingsWork in collaboration with other agency and program staff, in order to coordinate servicesCommunicate effectively and in a timely manner with management, peers, and clientsParticipate in evaluation and trainings in order to better respond to client/community needsOther duties as assignedQualificationsBachelor’s degree in related field, strongly preferred but not requiredMinimum of one (1) year of experience in case management requiredExperience and ability to demonstrate knowledge of issues faced by population served (at-risk, experiencing homelessness, and/or formally homeless)Knowledge of HMIS databases and the Coordinated Entry System, strongly preferredSkilled in non-violent crisis interventionAbility to self-motivate and be flexible in a fast-paced environment with minimal supervisionStrong interpersonal and team building skillsStrong knowledge of Microsoft Programs (Word, Excel, Outlook). *Computer skill test may be requested during the interview.
