AMB at Brookside Community Health located in the heart of Jamaica Plain Boston.Brookside is a licensed Health Center of Brigham and Women's Hospital and provides multi-disciplinary, comprehensive care with services in a range of medical areas including: Adult, Family and Pediatric Primary Care, OB/GYN, Dental/Oral Surgery, Nutrition/WIC, and Behavioral Health. Each year, more than 11,000 patients from Jamaica Plain and the city of Boston use these services, producing more than 69,000 visits to Brookside each year. The health center's services are provided primarily by a bilingual English/Spanish medical and professional staff.Brookside is a part of the Brigham and Women’s Hospital and has been providing services as a community health center for over 50 years. Our commitment to our patients and this community is evident in all we do.PRINCIPAL DUTIES AND RESPONSIBILITIES:A. Patient Care OrganizationProvides direct care, assessment/diagnoses, medication management, counseling, and teaching to a select patient population.Arranges for appropriate plan of care and follow-up based on outcome of diagnostic, lab, and assessment findings.Records pertinent patient care findings in the medical record.Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.Maintains interdisciplinary approach to planning and communicating patient care information by discussing patient data with appropriate physicians and other health care professionals.B. Quality of CareAssists in the development, implementation and revision of nurse practitioner guidelines and monitoring requirements for prescription writing in collaboration with their designated physician and Nurse Director.Evaluates outcomes of own patients’ care and makes necessary revisions in care plans.Participates in departmental and organization quality improvement program.C. CoordinationDemonstrates appropriate collaborative and consultative relationships with other providers within the institution and community.D. BudgetParticipates in setting productivity metrics and in cost effective management of personnel, time, supplies, and equipment in his/her area.E. EducationActs as preceptor for nursing students if applicable.Participates in orientation of new staff.QualificationsEducation:Master’s in Nursing (MSN), (or) DNP (or) PhD, in Psychiatric-Mental Health required.Require PMHNP (Psychiatry Mental Health NP) certification.Current licensure as a Nurse Practitioner in the Commonwealth of Massachusetts.Require active DEA license and a Massachusetts Controlled Substance Registration number.Experience:Minimum 2 years of current psychiatry nursing experience as a Registered Nurse or Nurse Practitioner required.Prefer Psych nursing experience in a community setting and/or working with a diverse patient population.Spanish language skills strongly preferred.Skills and Abilities Required:Expert clinical judgement skills to function in the independent role.Excellent interpersonal relationship and communication skills in order to collaborate with patients’ families, staff and community health workers.Working Conditions:Works in a variety of patient care environments where there may be exposure to communicable disease, and hazardous materials.Supervisory Responsibilities:May assume responsibility for providing direction to designated personnel.Brigham and Women’s Hospital is an Affirmative Action Employer.
