Job Description - Nurse Practitioner, Primary Care/Community Health (3302971)Nurse Practitioner, Primary Care/Community Health - (3302971)NURSE PRACTITIONER (M.S.)/ Full Time, 40 Hours/ Rotating Variable/ BWH Boston- Southern Jamaica Plain Health CenterSouthern Jamaica Plain Health Center (SJPHC) is a comprehensive, multi-disciplinary, community health center licensed by Brigham and Women’s Hospital (BWH). SJPHC serves a diverse population of over 11,000 individual patients and provides 68,000 clinical visits across four clinical departments each year, as well as provides access to specialty services. SJPHC has a racial justice and public health mission, to address the overall health and well-being of the residents of Jamaica Plain and its surrounding communities with a commitment to engaging in practices that promote racial justice, equity, and the elimination of all oppressive systems.GENERAL OVERVIEWThe Nurse Practitioner is an advanced practice registered nurse who has acquired specialized knowledge and skills through completion of a nurse practitioner program. The practitioner has the credentials to function in an expanded nursing role and is responsible for the daily management of selected patients. The practitioner has responsibility for assessment and management of patients, including diagnostic and therapeutic interventions, and continuous implementation and evaluation of an appropriate plan of care.PRINCIPAL DUTIES AND RESPONSIBILITIESA. Patient Care OrganizationProvides direct care, counseling, and teaching to a select patient population.Perform in-basket support to department providers and clinical team during time off / vacation/leaves.Performs physical examinations and provides preventive health measures appropriate to patient needs.Orders, interprets, and evaluates appropriate lab and diagnostic tests to assess patients’ clinical problems and health care needs.Arranges for appropriate plan of care and follow-up based on outcome of diagnostic, lab, and physical assessment findings.Records pertinent patient care findings in the medical record.Maintains interdisciplinary approach to planning and communicating patient care information.Incorporates the concepts of health maintenance, prevention, and promotion into daily practice through patient education and counseling.B. Quality of CareAssists in the development, implementation and revision of nurse practitioner guidelines.Evaluates outcomes of own patients’ care and makes necessary revisions in care plans.Participates in departmental and organization quality improvement program.C. CoordinationDemonstrates appropriate collaborative and consultative relationships with other providers.Serves on Nursing and Hospital Committees and assumes responsibility for disseminating and communicating information to staff.D. EducationActs as preceptor for nursing students and staff as requested.Participates in orientation of new staff.Education:Master's in Nursing (MSN) (or) DNP (or) PhD Degree in Nursing required.Completion of a Master’s or post Masters’ degree in

Family Nurse Practitioner (FNP) or Pediatric Nurse Practitioner (PNP)

program accredited by the Commission on Collegiate Nursing Education (CCNE) required.Valid prescriptive authority from both MA DPH and Federal DEA required.Current APRN License to practice in Massachusetts Required.Experience:Minimum 3 years’ recent experience as a Nurse Practitioner in a Primary Care setting required.Clinical experience managing both acute and chronic disease states and primary prevention required.Significant experience (>50% of primary care patients) with providing care to patients ages 0-22 required.Demonstrated experience working within an interdisciplinary care-team model required.Bilingual English/Spanish strongly preferred.Brigham and Women’s Hospital is an Affirmative Action Employer.
