United Hospice has an extraordinary opportunity for a skilled Registered Nurse with experience in providing pain management and acute care. As Hospital Liaison you provide both clinical and administrative oversight at your assigned hospital location for United Hospice (UH). Your responsibilities include developing a hospice presence in the hospital, attending and facilitating rounds and care conferences, including, conferring with all care healthcare providers with the primary purpose of increasing UH admissions, and performing hospice admissions as needed.

You will be expected to maintain the highest level of confidentiality concerning patient and family information. Bi-lingual (Spanish-English) is a plus! Qualifications:

Must have a valid and current NYS Registered Nurse license; BSN preferredTwo years recent clinical nursing experience with strong clinical skills including one year recent Med-Surg. experience requiredComplies with accepted nursing professional standards and practicesAbility to assess the needs of terminally ill patients and their families and to plan care to meet these needsCommits to take and pass the hospice and palliative care nursing certification exam (CHPN) within three years of hire if not currently certifiedCommitment to the Hospice Philosophy of CareAbility to relate to a diverse population and workforce in a personable and effective mannerAble to communicate effectively, while sustaining a caring, kind and professional dispositionMust have excellent written and verbal communication skillsProficient in MS Office Suite, EMRs and ability to learn new software systemsHospice admission and end-of-life care experience preferredMust have daily access to a vehicle with a valid driver's license/acceptable driving record and insurance for travelResponsibilities:

Identify hospitalized nursing home residents who meet eligibility criteria to arrange for or conduct admissions to UH before dischargeMust develop and maintain positive relationships with referral sources and promote exemplary customer service to physicians, facility staff, patients, and familiesServe as an integral member of our clinical team to work in conjunction with the hospital case managers in reviewing the hospital daily census to identify appropriate patients for UH admissionsAdmit patients to hospice as neededServe as covering visit nurse for hospitalized UH patients in absence of regular staffWork in conjunction with hospital case managers in reviewing the hospital daily census to identify appropriate patients, providing consultation and follow ups requestedIdentify and develop new opportunities for increasing referralsParticipate in on-call rotation to handle new referrals when office is closed
