Pediatric Hospice & Palliative Care Manager

that will be responsible for management, oversight, and coordination of the care and services of pediatric hospice and palliative care patients and their families referred to By the Bay Health’s pediatric team, By the Bay Kids.By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.Essential Duties & Responsibilities include

(but are not limited to):Management of the Pediatric Palliative and Hospice Programs under the direction of the Clinical Director.Collaborate with the Director of Patient Care Services to strategically grow the Pediatric program throughout the San Francisco Bay area.Participates in development of policies, procedures, protocols, and workflows aligned with anticipated programmatic growth.Partners with contracted vendors, hospitals, and facilities to establish strong referral pathways and ensure quality care.Receives and/or coordinates new patient referrals and assigns patient to appropriate staff member(s) to assure adequate patient care coverage.Schedules, facilitates, and provides leadership to IDT meetings, including facilitation of problem solving, compliance with visit frequency, and delivery of team support.Education/Experience:Minimum of two years of nursing experience or social work practice in a home health/hospice and/or palliative care setting within the past five years.Minimum of two years supervisory experience preferred, within the past seven years.Certificates, Licenses, Registrations:Current license as a registered professional nurse in the State of California, BSN preferred or Master’s in Social Work, LCSW preferred.HPNA certification preferred.Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.Pay Range:
