Palliative Care Nurse Case Manager
to work
4 days a week in Marin County with coverage to San Francisco County as needed.
This position is part of a multidisciplinary team providing specialized medical care for people with chronic and/or complex conditions. While focused on community-based palliative care, this role will be cross-trained in hospice care to ensure a seamless transition for patients who may need end-of-life services.By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.Essential Duties & Responsibilities include
(but are not limited to):Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions.Coordinate all patient/family services and prioritize needs with the members of the interdisciplinary team (IDT).Use a case management approach and refer to other services as needed.Coordinate the implementation of the plan of care and ensure care coordination for each patient.Education/Experience :One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing.Minimum of two years of professional nursing experience with at least two years in an acute care, home health or palliative care/hospice setting.Certificates, Licenses, Registrations:Current license as a registered professional nurse in the State of California.HPNA certification preferred.Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.Pay Range:$51.00 - $64.00 hourly
