Duties & Responsibilities
FUNCTIONAL DUTIES AND RESPONSIBILITIES
1- PATIENT CARE DUTIES AND RESPONSIBILITIES:
Directs the development, implementation and evaluation of the nursing regimen for patients / clients within an assigned unit involving the diagnosis and treatment of responses to actual and potential health problems by means of nursing methods and techniques such as case finding, health teaching, health counseling and the provision of care supportive, restorative and well-being.Coordinates the preparation and maintenance of nursing care plans and progress notes on patients/ clients

Assesses each patient needs and establishes appropriate plan of care based on Nursing Assessment and Diagnosis.Maintains standards of care by ensuring that an individualized plan of care for each patient is developed and implemented.Receives
incoming
and outgoing shift report from staff by making walking rounds and ensures continuity of care. Utilizes the electronic and paper medical records effectively.Evaluates patient's response to interventions and make appropriate modifications if needed and compares it with established standards. Provides appropriate follow-up action for identified problemsProvides PatienUFamily teaching based on patient's needs, ability, and readiness to learn. Explains, involves, and instructs patienUfamilies concerning the therapeutic regimen and environmental care.Serves as leader, role model, and patient advocate, clinical resource person for other nursing personnel and as an adjunct to staff development. Leads patient's therapeutic group efficiently.Conducts environmental rounds to assure compliance in Infection Control and Safety.Verbalizes awareness of cultural sensitivity in the work force.Ensures safe and secure environment free of hazards to reduce
the
risk of injury to patient, staff, and visitors including
MRI,
Fire Safety, and evacuation plan. Responds to and able to differentiate the clinical alarm systemDemonstrates competency in Code Management and responding to RRT. Reinforces infection control policies and proceduresII -
MULTIDISCIPLINARY TEAM RESPONSIBILITIES:Coordinates patient care by collaborating with multidisciplinary team to develop plans to meet the needs of patient and family.Initiates patient care rounds with physicians and participates with the team in developing priorities of patient care.Communicates effectively with the patient/family, medical, nursing, and other staff during treatment planning and implementation of the plan.Ill -
LEADERSHIP DUTIES AND RESPONSIBILITIES:Schedules, assigns, guides, direct and instruct nursing personnel in nursing activities.Monitors Quality improvement activities and ensures that staff collects data and submits written reports monthly.Supervises professional and non-professional personnel.Establishes realistic priorities and delegates tasks based on staff competence and ensures work is completed on allotted time.Makes
rounds to check accuracy of the following - charting, Nurse's notes, doctor's orders, patient care plan, narcotic sheets, code cart and emergency equipment daily, refrigerator temperature log.Reviews and analyzes trends pertinent to delivery of nursing care services within the unit and initiates appropriate innovations or makes recommendations for needed changes.Fosters good public relations for unit, nursing department, hospital, and community.Assists in recruitment, interviewing, and selection of staff for a unit.Acknowledges accomplishments of coworkers and other team members through written and verbal communication.Demonstrates initiative in identifying and resolving conflicts and seeks assistance as neededMaintains attendance profiles on personnel and reviews it periodicallyCounsels staff when indicated and makes recommendations for further corrective action.Completes time schedule for staffing and submits to the DON for approval and assists in developing staffing patterns for quality careDevelops and maintains a system of evaluation of nursing care, practices and procedures, including evaluation of nursing personnel within a unit.Completes performance evaluation for assigned staff.Participates in the development and in the implementation of unit orientation to all new employees.Identifies learning needs of staff and initiates process to meet these needs.Offers constructive suggestions for change in department/unit policies or proceduresConducts conferences to develop nursing care plans and to promote staff development.IV-

Licensed and currently registered to practice as a Professional Registered Nurse in New York State; and,Demonstrated competence as a Nursing practitioner; and,Demonstrated commitment to continued professional development, and,Three years of professional nurse experience; and, A Baccalaureate Degree in Nursing;Minimum Qualifications1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA); and3. One (1) year of experience as a registered nurse.
