Who We Are Looking For:

The Director of Care Coordination will oversee all aspects of NAMI Chicago’s targeted case management and navigation programs.

This includes the Clinical Support program that provides direct case management support and resource mobilization to highly acute mental health cases and our Mental Health Court program that assist clients with achieving their recovery goals as they navigate the justice system.

The position requires an individual with clinical experience in the mental health crisis continuum and high-level skills to independently and systematically develop, assess and implement NAMI Chicago’s intensive and targeted care coordination programs while supporting the implementation of strategic goals, clinically rooted frameworks, and theory across these programs.

The ideal candidate is an effective manager that will lead and guide the care coordination team leaders to accomplish the program’s objectives.

The Director reports to the Chief Program Officer

and will work closely with social service organizations, government partners, emergency response systems

and other stakeholders.

Essential Functions & Responsibilities:

Strategic Leadership

Provides guidance, develops the overall strategy, and maintains partnerships with external stakeholders involved in targeted care coordination programs, including government, public safety, and hospital system stakeholders.

Develop, audit and manage the clinical and operational functions of NAMI Chicago’s Mental Health Court, Clinical Support programs and other targeted case management programs.

Draft manuals, standard operating procedures, and other tools to disseminate clinical and operational requirements.

Assist Program Leadership in the development of strategic programmatic goals and work to achieve those goals.

Manage Senior Managers and additional middle management to ensure program expectations are being adhered to and monitored.

Support program evaluation by developing program goals and objectives, metrics for data tracking, and review of quality assurance working with Program Leadership.

Weekly review and supervision of program activities and direct supervision of management staff.

Compile reports as needed for executive leadership and board committees.

Execute programmatic budget development and management including official records and documents, and ensuring compliance with federal, state and local regulations.

Partners with NAMI Chicago Departments on coordination of services across the agency as they overlap.

Engage with NAMI Chicago’s Development Team on funding opportunities through grants and donors.

Partners with Senior Director of Integration in all quality assurance and program evaluation activities.

Maintains a working knowledge of significant developments and trends in the field of legal system, mental health, and recovery.

Actively participate in the agency’s Leadership Team.

Perform other duties as assigned.

Operations & Compliance

Develop, review and implement standard operating procedures to ensure program compliance across grant contracts, accrediting and certifying bodies and other funders.

Act as compliance manager for programmatic training and performance, assessing quality of program implementation, developing mitigation plans as needed, working in partnership with Senior Director of Integration on quality assurance program.

Align program operations with organizational goals.

Assume responsibility for adherence to developed standard operating procedures and maintain ongoing best practice development and audits related to those policies.

Report to Executive Leadership Team on program impact and quality.

Exercising Sound Judgement – Thoughtfully analyzes situations and challenges to arrive at sound decisions.

Time Management – Ability to prioritize tasks, manage time and complete projects in a fast-paced, changing environment with minimal supervision.

Qualifications & Requirements:

Education: Graduate Degree in Social Work, Counseling, or equivalent or related field required.

Experience working in the criminal court system or hospital systems preferred.

6+ years of experience in their field.

Extensive knowledge of recovery principles; experience working with individuals living with chronic mental illness and co-occurring disorder.

Bilingual in English and Spanish strongly preferred.

Computer Skills: Proficiency Microsoft Access, Excel, and Word. Proficiency in Internet Explorer, Outlook/Exchange; Windows operating systems; and other software routinely used by NAMI Chicago.

Compensation range begins at $90,000/year and is commensurate with experience.
